Our Care Process

1. Reach Out
The process begins when families, agencies, or professionals reach out to learn more about our services. You can contact us by phone or email to discuss the youth’s needs, background, and suitability for our program. This first step helps us understand how we can best provide support.
2. Referral Review & Discussion
Once an inquiry is received, our team carefully reviews the referral information. We connect with involved professionals or family members to gather additional details and determine whether our program is the right fit for the youth’s needs and level of support required.
3. Assessment & Planning
If the referral is suitable, we complete a detailed assessment of the youth’s needs, strengths, and challenges. Based on this, we develop a personalized care plan focused on stability, safety, and long-term development.
4. Admission & Placement
Once approved, the youth is welcomed into our home in a safe and supportive manner. We focus on building trust, establishing routines, and helping them adjust to their new environment with consistency and care.
5. Ongoing Care & Support
After placement, our team delivers structured, trauma-informed care including emotional, behavioral, educational, and life skills support. Regular reviews ensure the care plan continues to meet the youth’s evolving needs.
